To request access to records:

  • Complete Form C and submit it with a request fee to the head of the private body.
  • The form must provide sufficient details to identify the requested record(s), indicate the form of access required, and provide the requester’s postal address or fax number within South Africa.
  • The requester must identify the right they are seeking to exercise and explain why the requested record is necessary to protect that right.
  • If a request is made on behalf of another person, proof of the requester’s capacity must be submitted.
